This is a 45-year-old patient who presents an alteration in the growth of the facial bones, apparently his mandible is larger, however, already taking measurements of his facial skeleton, we realize that the maxillary bone, that is, the that has the upper teeth did not grow adequately anteroposteriorly giving that prognathic appearance, in which an advance of the maxillary bone was made to achieve facial balance and also to be able to reach an adequate bite
In this case, there is an underdevelopment of the maxilla and an excessive growth of the mandible, which led to the decision to perform a maxillary advancement and mandibular setback, achieving a proper facial balance. This type of procedure is known as double jaw surgery and is part of the orthognathic surgery specialty.
The patient was diagnosed with mandibular prognathism, meaning the mandible has excessive growth in the anteroposterior direction. Orthognathic surgery was performed to reposition the mandible backward.
When the maxillary bone does not grow enough in the anteroposterior direction, it results in a straight face with depressed lateral portions near the nose, affecting facial balance. In this patient, a maxillary advancement was performed, completely changing the face's shape and achieving a beautiful facial harmony.
The patient underwent hemimandibulectomy due to mandibular osteosarcoma without immediate reconstruction. We decided to perform a microvascularized fibula bone graft, connecting the artery and vein to the facial blood vessels. Subsequently, dental implants were placed in the grafted bone, followed by an implant-supported full denture.
